As part of Mount Sinai's mission to provide patients with the best clinical care, we are pleased to announce that we will soon be upgrading our electronic health record systems at your doctor's office. As part of this upgrade, starting Tuesday, November 10th, 2015, access to your health information will be available through MyMountSinaiChart .

What is MyMountSinaiChart?
MyMountSinaiChart is a free tool that allows you to view and print portions of your health record, view test results, manage and request appointments, communicate with your physician(s), request medication refills, and more. MyMountSinaiChart also allows you to view a current summary of your medications, allergies, medical conditions, and immunization histories.

Why am I receiving this message?
You are receiving this message because according to our records, you currently have an active MedicsDocAssistant Patient Portal account, and you have been a patient at the location listed above.

I currently use MedicsDocAssistant Patient Portal to view my records. Does this apply to me?
Yes. Beginning on Tuesday, November 10th, 2015 you will no longer be able to see the practice listed above when you log into the MedicsDocAssistant Patient Portal. However, you will be able to use MyMountSinaiChart to see this practice instead.

Do I need to take action?
No. There is no action required of you.

How will I get access to MyMountSinaiChart?
For security reasons, access to MyMountSinaiChart requires a registration code which can only be obtained from your doctor's office in-person at your next appointment. Registration codes cannot be given over the phone, nor can they be requested prior to Tuesday, November 10th, 2015.

The Patient Portal is not to be used for urgent or emergent issues. If your need is urgent, please call your provider's office directly. In the event of a medical emergency, call 911.
